# Creating Shop-Level Consent Templates

Create a reusable form in your shop workspace. Use the written-form editor to add your shop's text, then save and reopen it before using it with clients.

## Before you start

You need access to your shop's **Waivers** page. Check the workspace name at the top of the screen. Prepare the wording your shop wants to use and choose the service it applies to.

## Create and check a template

1. In your shop workspace, open **More**, then find **Waivers** under **Content**.
2. Select **Create new template**. Choose **Write in Apprentice** to use the text editor.
3. Open **Form details**. Enter the **Template title**, choose the template type, and select a **Service**. Choose **Any service / service-agnostic** only when the content applies across services. Select **Done**.
4. Write your content in the editor. Use the toolbar for headings, lists, and other formatting. Select **Preview** to check how the content looks.
5. Select **Save**. Find the template by its title, then select **Edit** to check that your text was saved.

6. Check the wording and any fields before marking the template as reviewed. Saving a new template can leave it marked **Needs review**. A saved template is not proof that a client has received or completed it.
7. When you are ready to use the form, follow [Setting Default Preparation Requirements](https://help.useapprentice.com/en/article/setting-default-preparation-requirements-1nw8ps8/). Check the client flow before relying on the form for appointments.

## Edit an existing template

Search for the title on **Waivers**, then select **Edit**. The editor includes an optional change summary. Save your changes and reopen the template to check them.

Keep completed, signed documents as records of what the client agreed to. Do not treat an edited template as a replacement for an earlier signed document.

## If something goes wrong

- **The template is missing:** Clear search and service/type filters, then search for its title again.
- **The page crashes after saving in Markdown mode:** Reload the page and search for the template before trying again. If no template was saved, try **Write in Apprentice** with the normal text editor. Do not assume a failed save created a template.
- **The form has not reached a client:** Check its review state and the shop's default preparation settings. Saving alone does not confirm delivery.
